Dr. Andrew Jacono Reviews New Trends from Brazilian-American Aesthetic Meeting

New technology was on display at this years 6th American-Brazilian Aesthetic Meeting, held in Florianopolis, Brazil February 23-27, but the latest aesthetic trends held the spotlight throughout the conference as well. Andrew Jacono, M.D., F.A.C.S., director of the New York Center for Facial Plastic and Laser Surgery, reviews the latest and greatest from the gathering of top docs from around the globe.

VeinViewer was among the newest technology highlighted during the four-day meeting. The mobile vascular imaging system allows health care providers to clearly see accessible vasculature as a real-time HD image, directly on the surface of the skin. While nicking a vein during the injection of fillers and other cosmetic facial treatments can cause unsightly bruising, VeinViewer may help prevent this unwanted side effect.

Even the most skilled injector isnt able to avoid bruising, as theres no way to pin down the exact location of veins by sight alone, says Dr. Jacono. The use of this new imaging system will have the greatest impact on injections done around the eyes, where veins are commonly clustered and bruising tends to occur most often.

As for key trends, stacking treatments was among the hot topics. According to Dr. Jacono, the thinking is moving away from using either injectables or surgery, and toward using one to complement the other. Even after surgery, injectables are now often used to maintain and enhance the results, and vice versa.

Injectables and surgery are no longer thought of as separate specialties; layering treatments has become the new normal, says Dr. Jacono. Patients are starting to volumize the face in their late 30s and early to mid-40s, but at a certain point, the skin starts to loosen naturally with age. Whereas in the past patients would continue using volumizing fillers to the point of overfilling, now were encouraging the addition of tightening treatments like Ultherapy or mini-facelifts, followed by the continued use of fillers to help maintain volume of the face with age. Tightening the face doesnt help with volume loss, and filler doesnt help tighten, so some combination of the two is ideal.

Dr. Jacono also recommends layering a fractional CO laser resurfacing treatment to treat fine lines and wrinkles on the surface of the skin. His unique Fusion Therapy is a prime example of stacking treatments to achieve natural-looking results. Fusion Therapy combines three procedures platelet-rich plasma injections, Ultherapy and fractional skin rejuvenation that treat all layers of the face to address volume loss, uneven skin tone and wrinkles.

The shift in fat grafting to the use of thinner layers to restore youth and regenerate the skin marked another notable discussion topic at this years conference. While thicker layers have traditionally been used in fat grafting procedures to enhance volume, thinner layers of structural fat under the skin help restore support and over time can help improve the texture, tone and quality of the skin. These thin layers have also shown promise in the healing of acne and surgical scars, and may also help improve the appearance of sun damage.

Yet another hot topic among this years attendees was the recent shift from a heart-shaped face to the inverted egg. While the heart shape remains the standard of youthful beauty, it is no longer seen as the ideal in older patients, says Dr. Jacono. For patients in their 40s, the fuller, more rounded shape of the inverted egg accounts for some of the natural fall that comes with age, while still maintaining the semblance of youth.
