What is an arrhythmia? An arrhythmia (also called dysrhythmia) is an irregular or abnormal heartbeat. Your pulse indicates your heart rate, or the number of times your heart beats in one minute. Pulse rates vary from person to person. Your pulse is slower when you are at rest and increases when you exercise, since more oxygen- rich blood is needed by the body during exercise. You can tell how fast your heart is beating by feeling your pulse. You can feel your pulse on your wrist or neck. Place the tips of your index and middle fingers on the inner wrist of your other arm, just below the base of your thumb. Or, place the tips of your index and middle fingers on your lower neck, on either side of your windpipe. Press lightly with your fingers until you feel the blood pulsing beneath your fingers. You may need to move your fingers around slightly up or down until you feel the pulsing. You can count the number of beats in 10 seconds and multiply by 6 to determine your heart rate in beats per minute. A normal heart rate, at rest, is 50 to 100 beats per minute. Your doctor usually can diagnose a cardiac rhythm disturbance based on your symptoms, medical history, risk factors, a physical exam, and with results from tests and procedures. During a physical examination, your doctor will check your heart rate and rhythm, together with your pulse. If your doctor believes you may have a cardiac rhythm disturbance, he or she likely will order anelectrocardiogram(EKG) for confirmation. There also is a more advanced version of an EKG where your doctor may ask you to wear an event (or Holter) monitor. This is a portable device that is the size of a smartphone or smaller and can be worn under clothing, similar to an exercise heart rate monitor. This device will relay information back to your doctor and identify any abnormal rhythms that might happen intermittently, but not all of the time. The monitor usually is worn for 24-48 hours to get the required information. Your doctor also may recommend one or more of the following: echocardiogram (echo), chest x-ray, blood tests, stress test, and coronary angiography. The intended outcome of this RFP would have been provision of private psychiatric inpatient services to individuals appropriately referred by DBHDD or its agent in regions 1, 4, and 6 within the state of Georgia. The current contracts expire at the end of this calendar year. The department has canceled the RFP and will re-release a new RFP in the fall of 2018. Tyke was developed based on a request by Pediatric Cardiovascular Surgeons to deliver an ECM material that maintained the biomechanical properties found in our existing products, but was thinner, more pliable and better suited for intracardiac and branch pulmonary artery use in neonates and infants.1 Tykeis intended for use in neonates and infants for the repair of pericardial structures, as an epicardial covering for damaged or repaired cardiac structures, as a patch material for intracardiac defects, septal defect and annulus repair, suture-line buttressing, and cardiac repair. Tykemeets the same biomechanical requirements (tensile strength, suture retention strength and burst strength) as our existing ECM for Cardiac Tissue Repair, and bench testing has shown that Tykeexceeds the specified tensile strength, suture retention strength, and burst strength required for its intended surgical use.2 To assess the biological performance of Tyke, patches were implanted in the descending aorta and pulmonary artery of a growing lamb model.3 Explanted patches were evaluated by an independent, board-certified pathologist. Indications Cardiac rehabilitation initially was designed for low-risk cardiac patients. Now that the efficacy and safety of exercise have been documented in patients previously stratified to the high-risk category, such as those with congestive heart failure (CHF), the indications have been expanded to include such patients. Exercise training benefits persons with the following cardiac conditions: Recent myocardial infarction Cardiac transplantation [21] Exercise prescription depends on the results of exercise testing, which often includes cardiopulmonary exercise (CPX) testing. Patients with limitations due to chronic obstructive pulmonary disease (COPD), peripheral vascular disease (PVD), stroke, and orthopedic conditions still can be trained in the exercises through special techniques and adaptive equipment (eg, use of arm-crank ergometer). Cardiac rehabilitation services are contraindicated in patients with the following conditions: Uncompensated heart failure Severe ischemia, LV dysfunction, or arrhythmia during exercise testing Poorly controlled hypertension Hypertensive or any hypotensive systolic blood pressure response to exercise Unstable concomitant medical problems (eg, poorly controlled or "brittle" diabetes, diabetes prone to hypoglycemia, ongoing febrile illness, active transplant rejection) [22] In such patients, every effort should be made to correct these abnormalities through optimization of medical therapy, revascularization by angioplasty or bypass surgery, or electrophysiologic testing and subsequent antiarrhythmic drug … Continue reading →
