This report describes the latest concepts of pathomechanisms of pain as a basis for management and development of new pharmacotherapies for pain. Major segments of the pain market are arthritis, neuropathic pain and cancer pain. Because pain is a subjective sensation, it is difficult to evaluate objectively in clinical trials. Various tools for pain measurement are described, including brain imaging. Most of the currently used analgesic drugs fall into the categories of opioids and nonsteroidal antiinflammatory drugs such as COX-2 inhibitors. Non-opioid analgesics include ketamine, a N-methyl-D-aspartate receptor antagonist. Adjuvant analgesics include antidepressants and antiepileptic drugs used for the treatment of neuropathic pain. Management of pain is multidisciplinary and includes both pharmacological and non-pharmacological methods such as acupuncture, transcutaneous electrical nerve stimulation and surgery. Various pain syndromes require different approaches in management, for example, the main category of drugs for migraine are triptans such as sumatriptan. Here are details of some of the drugs commonly used in sports doping. ERYTHROPOIETIN (EPO) * EPO is a peptide hormone that is produced naturally by the human body. It is released from the kidneys and acts on the bone marrow to stimulate red blood cell production. By injecting EPO, athletes aim to increase the concentration of red blood cells to boost their aerobic capacity. * EPO abuse has serious health risks. By thickening the blood, EPO abuse can lead to an increased risk of heart disease, stroke and cerebral or pulmonary embolism. The drug has been implicated in the deaths of several athletes. The 78 people arrested by Florida deputies between Jan. 10 and 13 all have one thing in common: they're all suspects in a massive prostitution investigation. Undercover deputies with the Polk County Sheriff's Office spent just four days arresting the suspects, targeted through websites used to solicit prostitution, according to a press release."* Since all other crimes have apparently been solved, the Polk County Sheriff has cracked down on prostitution, getting "every thug" in the county behind bars. Study: Hundreds of rare disease drugs in testing

WASHINGTON (AP) -- For the nearly 30 million Americans who suffer from a rare disease, there's heartening news: Drugmakers have been shifting much of their research away from pills for the millions to uncommon disorders that often kill prematurely because there are few or no treatment options. Pharmaceutical and biotech companies are running patient tests of more than 5,400 potential new medicines, including many being tested for multiple conditions. Nearly 1,800 research projects are for rare diseases, and hundreds more are for disorders for which there's been no new medicine in a decade or more. That's according to a study to be released Thursday, sponsored by the trade group Pharmaceutical Research and Manufacturers of America, known as PhRMA. The study also found that thousands of those experimental drugs could be the first in a new medicine class with a unique target or effect, including 45 percent of 1,100 drugs that made it to the final patient testing stage. At least 577 use new technologies, from gene therapy to cloned antibodies, to precisely target the disease site and limit side effects elsewhere. Currently, antiretroviral therapy is given only once the immune system has been seriously weakened by infection. A trial, in the New England Journal of Medicine, showed that a year-long course of therapy after diagnosis helped preserve the immune system and keep the virus in check. It is thought that early treatment may also reduce the spread of HIV. The virus is no longer a death sentence for patients who get the best care and drugs. Treatment is given once their CD4 T-cell count, a part of the immune system, falls below 350 cells per cubic millimetre of blood. However, there has been some speculation that starting as soon as a patient is diagnosed may be more beneficial.
