Objectives We live in a society that is slowly getting older. Two hundred years ago, 30% of women lived long enough to reach menopause, whereas 90% of today's women will experience the climacteric.1 Although menopause is not a disease, it is a relatively estrogen-deficient state. There are many consequences of a relative lack of estrogen that may affect health. Estrogen replacement therapy (ERT) and combination estrogen and progesterone therapy, also known as hormone replacement therapy (HRT), may ameliorate some adverse effects, but may in turn increase other risks. Menopause is diagnosed by the findings of at least 6 months of amenorrhea in a woman over age 40 or amenorrhea combined with a FSH level above 40 pg/ml. Normal women experience ovarian failure at a mean age of 51 years, with 95% becoming menopausal between the ages of 45 to 55 years.2 Estrogen is the most effective treatment available for relief of the menopausal symptoms that many women experience.2 For the past three decades, long-term (>5 years) estrogen and combined estrogen-progestin therapy (HRT) have been routinely prescribed for prevention of coronary heart disease (CHD) and osteoporosis, based upon extensive observational data demonstrating a striking protective effect of estrogen on the heart … Continue reading

Hormone replacement therapy can be administered through a skin patch or pill. istockphoto Most women in menopause who opt to take hormone replacement therapy (HRT) to manage symptoms that the "change of life" brings know there are risks that come with these drugs. While the hormones --synthetic estrogen and progestin -- can work magic to stop hot flashes, decades of research has identified a strong link between HRT and conditions such as heart disease, stroke and breast cancer. Now a new large scale study may add ovarian cancer to this list. An analysis of 52 studies involving 21,488 women, published Thursday in The Lancet, finds even short-term use of these drugs correlated with a higher risk for ovarian cancer -- as much as 40 percent higher than women who don't take hormones. The study is based on all prior research that investigates the connection. "This is the first big study like this to really come out and say ovarian cancer is an issue," Dr. Taraneh Shirazian, an assistant professor of obstetrics, gynecology and reproductive science and a minimally invasive gynecological surgeon, told CBS News. The new study found that when women used hormone replacement therapy for less than five years after menopause, the risk of ovarian cancer increased by about 40 percent. "We have evidence, proof, that there is a small but real excess risk of cancer of the ovaries with hormone therapy use," said study researcher Sir Richard Peto, professor of medical statistics and epidemiology at the University of Oxford, in England. Peto said the increased risk was significant from a statistical standpoint but emphasized that the risk is a small one. It would mean that for women who take hormone therapy for five years from around age 50, one extra ovarian cancer diagnosis for every 1,000 users would be expected, and one extra ovarian cancer death for every 1,700 users, the study found. It's also important to note that the current study wasn't designed to definitively show a cause-and-effect relationship between hormone replacement therapy and ovarian cancer. Taking hormone replacement therapy (HRT) for menopause, even for just a few years, is associated with a significantly increased risk of developing the two most common types of ovarian cancer, according to a detailed re-analysis of all the available evidence, published in The Lancet. The findings from a meta-analysis of 52 epidemiological studies, involving a total of 21488 women with ovarian cancer, almost all from North America, Europe and Australia, indicate that women who use HRT for just a few years are about 40% more likely to develop ovarian cancer than women who have never taken HRT. "For women who take HRT for 5 years from around age 50, there will be about one extra ovarian cancer for every 1000 users and one extra ovarian cancer death for every 1700 users", explains study co-author Professor Sir Richard Peto from the University of Oxford in the UK. Although HRT use fell rapidly about a decade ago, this decrease has now levelled off and in the UK and USA alone about 6 million women are still taking HRT. Women who undergo hormone replacement therapy (HRT) have a significantly increased risk of developing ovarian cancer, according to a major new study. Researchers from the University of Oxford analysed 52 previous studies involving 21,000 women found that even those who took it for less than five years raised the risk level, although it reduces once they had stopped. The researchers estimated one million women in Britain use HRT to relieve symptoms of the menopause, and therefore, the link could affect hundreds of women each year there. The study, published in the Lancet medical journal, has led to calls for medical guidance on HRT to be updated given the causal relationship and the Medical and Healthcare Products Regulatory Authority (MHRA) said it would look at the findings. The risk of cancer falls after women stop hormone replacement therapy Hormone replacement therapy significantly increases the risks of women suffering the two most common forms of ovarian cancer if it is taken by over-50s for up to five years, according to a major global study published today. In the analysis of more than 50 studies from North America, Europe and Australia, published in the Lancet, women who use HRT for just a few years are about 40 per cent more likely to develop ovarian cancer than women who have never undergone HRT. Current WHO, US and European safety guidelines do not specifically warn about the dangers of ovarian cancer from HRT treatments, which are used by six million people in the UK and US. In Ireland, the Irish Medicines Board has said the benefits of HRT outweigh the risks for menopausal women, but minimum effective dose should be used for the shortest duration. However, the risks are unfavourable as a first-line treatment for osteoporosis, it says.
