Risks of HRT

HRT/ERT has undergone a pendulum like effect in its popularity. When it was first introduced, it was thought to be a wonder drug and then the association with endometrial cancer was recognized. The addition of progestin to the regimen minimized this risk and it was again very popular. Recent studies have shown an association with breast cancer and an increased risk of other conditions resulting in a more conservative approach to the prescribing of this treatment. The absolute risk of HRT in women in their 50s with use for less than 5 years is low and those who have been menopausal for >10 years or in their 60s are at greater risk. The increased risks noted here involves the use of HRT for > 5years. Breast cancer is the most common cancer in women and the news that HRT increased this risk was the most shocking revelation of recent large studies. The risk was increased by 6.8 additional cases per 1000 women after more than 5 years of use of combined estrogen and progestin therapy. What can you expect if you quit? If you are healthy, most experts agree that HRT is safe to use at the lowest dose that helps for the shortest time needed. If you're 59 or older, or have been on hormones for 10 years, you should talk to your doctor about whether it's time to quit. Here are some things to consider when you decide. A large study called the Women's Health Initiative showed that women who took estrogen and progestin were more likely to have illnesses such as heart disease, breast cancer, stroke, and blood clots than women who didn't. Some women in the study took estrogen only because they had had hysterectomies to remove their uterus. They had a slightly higher chance of stroke and blood clots, but not breast cancer or heart disease. The risks depend on your age when you started hormones and how long you've taken them. Testosterone use among men has increased significantly in recent years. Much of this increase is in response to doctors recommendations for treatment of symptomatic low testosterone levels, advertising of testosterone replacement therapy and the belief that low testosterone may increase the risk of developing cardiovascular disease (heart attack and stroke). Most recent studies that followed a population over time suggested that deficiencies in male hormones such as testosterone and dihydrotestosterone may increase the risk of cardiovascular disease. Because low testosterone is a potentially reversible condition, it makes some sense that hormone replacement would be a good idea to prevent cardiovascular disease. Additionally, higher testosterone levels among men in these observational studies was associated with a healthier cardiovascular risk profile, such as less diabetes, lower blood pressure, lower LDL cholesterol (bad cholesterol) and higher HDL cholesterol (good cholesterol). However, reality may tell a different story. We had long believed that hormone replacement therapy in women would protect against heart attacks and strokes. Estrogens help lower cholesterol and have other beneficial effects. By Barbara Bronson Gray HealthDay Reporter Latest Womens Health News MONDAY, May 5, 2014 (HealthDay News) -- A comprehensive look at the decade-old "estrogen study" -- the U.S. government-sponsored trial that turned the tables on assumptions about hormone replacement therapy -- suggests there can be a billion-dollar upside to the millions spent on research. The research, published May 6 in the Annals of Internal Medicine, shows the public has received a high return on its tax-funded investment in the Women's Health Initiative (WHI). "Everything we do in medicine comes with a level of uncertainty," said Dr. Scott Ramsey, the study's lead author and director of the Hutchinson Institute for Cancer Outcomes Research, in Seattle. "The question is whether it's worth the investment in research to be sure we're not making a mistake." The results of his research, he said, showed that, even though the WHI cost $260 million, the net economic return was $37 billion, or $140 for every dollar spent on the trial. May 6, 2014 News Review From Harvard Medical School -- Report: Hormone Study Returned Big Savings A landmark study that found hormone therapy may harm rather than help women after menopause cost the U.S. government $260 million. But the study results transformed medical practice. And that saved the health care system $37 billion, a new analysis concludes. That's $140 for every dollar spent. The National Institutes of Health funded the study, called the Women's Health Initiative. Older women, average age 63, were randomly assigned to receive either hormone treatment or placebo (fake) pills. The study was halted early, in 2002. Women taking estrogen plus progesterone had higher rates of heart attack, stroke, blood clots and breast cancer than those who got the placebo. There was no effect on heart attack or breast cancer rates in a second group of women who were given estrogen only. But they also had higher risks of stroke and blood clots. Hormone use for menopause symptoms dropped by half right away. It continued to fall in later years. Cases of breast cancer, heart disease and blood clots also declined.
