Estrogen Could Save Lives

Getty Images / Getty Images For more than a decade, doctors have cautioned women about the risks associated with hormone replacement therapy. But those warnings may have put one group of women at increased risk of dying early, according to the latest study. Researchers at Yale University say that nearly 50,000 women may have died prematurely after they stopped taking hormone replacement therapy (HRT) to treat menopause symptoms, following a much-publicized 2002 study that revealed the treatment increased risk of heart disease and breast cancer. The 2002 Womens Health Initiative(WHI) study, a 15 year investigation into the factors that contribute to the health of post menopausal women, was stopped three years early when a preliminary review of the data showed that women taking the combination of estrogen and progestin had a higher rate of breast cancer, heart disease and stroke than women taking a placebo. The results stunned both the public and the medical community, since doctors had been prescribing the hormones not just to treat menopausal symptoms such as hot flashes, but for extended periods of time to protect women against heart disease. Hormone replacement debate heats up again

The news arrived like a hot flash. Results from the first reported leg of the giant Womens Health Initiative study in July, 2002 had an immediate, staggering effect on one of the most established therapies in medicine. The study, involving some 16,000 postmenopausal women across the U.S., showed that those taking standard hormone replacement therapy, or HRT, had a 26 per cent greater risk of developing breast cancers than those given a placebo. It also showed a 41 per cent increase in strokes, a 29 per cent rise in heart attacks and a doubling in the risk of blood clots among the studys hormone replacement cohort. The trials were halted early because their findings were so distressing. Immediately, the use of HRT to combat the hot flashes, restless sleeps, vaginal atrophy and muddled minds that can accompany menopause plummeted around the world. And that, it seemed, would be that. Today, many in the medical community are questioning those seminal and settled health initiative findings and pushing to rehabilitate HRT for the indisputable benefits that the drugs can bestow. After a decade of rancorous debate over the risks and benefits of menopausal hormone therapy, experts from more than a dozen top medical organizations worldwide have finally come to something resembling a consensus. What did they decide, and why are women still baffled? What is menopausal hormone therapy? Also called hormone replacement therapy, its a treatment for women in the throes of menopause, the inevitable period of life when estrogen naturally declines. Taking estrogen or a combination of estrogen and another hormone, progestin, the synthetic form of progesterone, can alleviate some of the symptoms of menopause, including hot flashes, night sweats, insomnia and vaginal dryness, said Atlanta endocrinologist Scott Isaacs. Women typically hit menopause around age 51. Why is hormone therapy controversial? Hormone therapy has been under intense scrutiny since 2002, when a large study called the Womens Health Initiative reported that taking additional hormones specifically the combination of estrogen and progestin increased the risk of blood clots, stroke, breast cancer, heart attacks and gall bladder disease for some women.
