A stunning early termination of a study in 2002 on hormone replacement therapy (HRT) which erroneously indicated that HRT caused heart attacks and breast cancer, led both doctors and their patients to abandon the treatment worldwide. The real cause of the trial termination was for other reasons not related to any serious side effects or harm from HRT. However, the damage was done and women today still tend to believe HRT is an unsafe treatment. In a new study to be published in the journal Climacteric, concerns about HRT are discussed and the long-held beliefs are challenged. A provocative new study looking at postmenopausal women who underwent coronary artery calcium (CAC) scanning indicates that hormone replacement therapy (HRT) use is associated not only with lower CAC scores but also lower all-cause mortality over a mean follow-up of 8 years. The study, say investigators, should reopen the question of whether hormone replacement, according to contemporary guidance, may be useful in the primary prevention of cardiovascular disease. This is not a closed issue, senior author Daniel Berman, MD (Cedars-Sinai Medical Center, Los Angeles, CA), told TCTMD, noting that baseline amounts or changes in atherosclerosis may be useful in determining a role for HRT. Further studies should be performed, he said. The study by Berman and colleagues will be presented by first author Yoav Arnson, MD (Cedars-Sinai Medical Center), at the upcoming American College of Cardiology (ACC) 2017 Scientific Sessions and was released early to the media this week. The facts that most women and clinicians consider in making the decision to use, or not use, HRT are frequently wrong or incorrectly applied says Professor Robert D Langer in a paper entitled The evidence base for HRT: what can we believe?, which will be published in the forthcoming April edition of the journal of the International Menopause Society, Climacteric. It raises serious questions about the facts that have led women and their doctors to believe hormone therapy (often called HRT) is unsafe. The incendiary reports surrounding the early termination of the Womens Health Initiative (WHI) clinical trial of a specific form of HRT (conjugated equine estrogens with medroxyprogesterone acetate), in 2002, were highly misleading. They indicated that the study was stopped because HRT caused breast cancer and heart attacks, when in reality the study was stopped for more subtle reasons and there was no statistically significant harm for either breast cancer or heart attacks.
