Already, scientists in laboratories across the world have begun dipping mature cells in acid, hoping to see whether this simple intervention really can trigger a transformation into stem cells, as reported by a team of Boston and Japanese researchers last week. At the Harvard Stem Cell Institute, a number of scientists have already embarked on the experiment, which theyre informally calling stem cell ceviche, comparing it to the Latin American method of cooking seafood in lime and lemon juice. At meetings with other experts and even in casual conversation, stem cell scientists say they are exchanging surprise, doubt, and wonder about the discovery, reported in two papers in the journal Nature. The range of responses varies widely. But most scientists seem to be surprised and skeptical about the technique, though also impressed by the rigorous testing that experts in the field did on the cells. It appears that no one knows quite what to think. The Orthobiologic Institute (TOBI) alumni , Thomas Vangsness Jr M.D., recently published a ground-breaking article examining the intra-articular application of Mesenchymal Stem Cells (MSCs) following partial medial meniscectomy, a common knee surgery where part of the damaged meniscus is removed on the inner portion of the knee joint. This study examined the effects of MSCs on post-operative regeneration of meniscus tissue and changes in Osteoarthritis symptoms. The study consisted of 55 patients who underwent a partial medial meniscectomy, and received one of three different intra-articular kneeinjections 7-10 days post-operatively. Patients received either a high concentration of MSCs, low concentration, or a control. The patients were followed for 2 years and assessed for overall clinical outcomes, meniscal regeneration, and safety.The study revealed significantly increased meniscal volume (>15% increase), quantified via MRI, in both the high concentration and low concentration groups. However, more meniscus growth was seen the low concentration group. No patients in the control group reached the clinically significant 15% increase in meniscal volume. Furthermore, patients with existing osteoarthritis, experienced a reduction in pain compared the control group. Multiple sclerosis (MS), an inflammatory autoimmune disease affecting more than one million people worldwide, is caused by an immune reaction to myelin proteins, the proteins that help form the myelin insulating substance around nerves. Demyelination and MS are a consequence of this immune reaction. Bone marrow mesenchymal stem cells (MSCs) have been considered as an important source for cell therapy for autoimmune diseases such as MS because of their immunosuppressive properties. Now, a research team in Brazil has compared MSCs isolated from MS patients and from healthy donors to determine if the MSCs from MS patients are normal or defective. The study will be published in a future issue of Cell Transplantation but is currently freely available on-line as an unedited early e-pub. "The ability of MSCs to modulate the immune response suggests a possible role of these cells in tolerance induction in patients with autoimmune diseases, and also supports the rationale for MSC application in the treatment of MS," said study corresponding author Dr. Gislane Lelis Vilela de Oliveira of the Center for Cell-Based Research at the University of Sao Paulo. Researchers at Erasmus University Medical Center Rotterdam, The Netherlands, led by Dr. Qiuwei Pan and Dr. Luc van der Laan, have discovered that spontaneous tumorigenic transformation of human mesenchymal stem/stromal cells (MSCs) can occur during cell culture expansion, although the frequency is relatively low and often only observed after extensive passage in culture. This report appears in the January 2014 issue of Experimental Biology and Medicine. Currently, MSCs are being widely investigated as a potential treatment for various diseases. According to ClinicalTrials.gov, over 350 clinical trials using MSCs have been registered by the end of 2013 (with a search of: mesenchymal stem cells). For cell transplantation, MSCs are often isolated from either the patient or from a third party donor, and then expanded in cell culture before therapeutic application. In fact, spontaneous transformation of primary cells in cell culture has been well-investigated over decades. Malignant transformation of murine and monkey MSCs has also recently been reported. The current study confirmed that spontaneous tumorigenic transformation of human MSCs can occur during cell culture expansion.
