However, the optimal strategy of antiplatelet therapy after PCI remains debated. The randomized, controlled GLOBAL LEADERS study found that post-PCI, ticagrelor monotherapy (a P2Y12 inhibitor) following one month of dual antiplatelet therapy (DAPT) with aspirin was safe, but not superior to conventional DAPT in reducing the composite of all-cause mortality or new Q-wave myocardial infarction (MI). PHILADELPHIA The investigational drug apabetalone (Resverlogix) did not reduce the risk for major adverse cardiovascular events (MACE) in BETonMACE, a phase3 trial of patients who had recent acute coronary syndrome (ACS), type2 diabetes, and low HDL cholesterol. But this might be because the trial of this novel agent which acts at the epigenetic level, turning genes on and off was underpowered, both the lead author and an outside expert suggest. Apabetalone inhibits bromodomain and extra-terminal (BET) proteins, which are epigenetic transcription modulators of inflammation, thrombogenesis, and lipoprotein lipid metabolism. Continue reading
