Parkinson's disease facts Parkinson's disease is the second most common neurodegenerative disorder and the most common movement disorder. It is characterized by progressive loss of muscle control, which leads to trembling of the limbs and head while at rest, stiffness, slowness, and impaired balance. As symptoms worsen, it may become difficult to walk, talk, and complete simple tasks. The progression of Parkinson's disease and the degree of impairment vary from individual to individual. Many people with Parkinson's disease live long productive lives, whereas others become disabled much more quickly. Premature death is usually due to complications such as falling-related injuries or pneumonia. Most individuals who develop Parkinson's disease are 60 years of age or older. Since overall life expectancy is rising, the number of individuals with Parkinson's disease will increase in the future. Adult-onset Parkinson's disease is most common, but early-onset Parkinson's disease (onset between 21-40 years), and juvenile-onset Parkinson's disease (onset before age 21) also exist. Most medications for Parkinson's disease treat the declining levels of dopamine by either: You may start out with one medication, but over time your doctor will most likely add and change medications as your symptoms change. MAO-B inhibitors preserve dopamine. MAO-B is an enzyme that breaks down the dopamine in your brain. MAO-B inhibitors help prevent dopamine from being broken down, so more of your brain's dopamine is preserved. Dopamine agonists mimic dopamine. Dopamine agonists act like dopamine in the brain, mimicking the effects of dopamine. Levodopa helps replace dopamine. There is also a search for treatments that can slow the progression of the disease. Drug therapy to both manage the symptoms of PD, and slow the progression of the disease, remains the Gold Standard treatment option for PD. Levodopa increases the level of dopamine in the brain and thus relieves the movement problems of PD. Levodopa however has side effects. One of Levodopas common side effects is nausea so it is usually given with a second drug, carbidopa, that prevents nausea. The combination of Levodopa/Carbidopa effectively treats the symptoms of PD, allowing many people with PD to live relatively normal, active, and productive lives for many years. Dopamine Agonists are a class of drugs that, like levodopa, increase dopamine activity in the brain. Commonly prescribed dopamine agonists include: bromocriptine (Parlodel), pergolide (Permax), pramiprexole (Mirapex), and ropinirole (Requip). UC San Francisco scientists have discovered a possible mechanism for how deep-brain stimulation (DBS), a widely used treatment for movement disorders, exerts its therapeutic effects. Few medical treatments show results as rapid and dramatic as those seen with DBS, in which surgically implanted devices deliver electrical pulses to inner brain structures involved in movement. In most Parkinson's disease (PD) patients who receive the treatment, symptoms of slow movement, tremor, and rigidity sharply diminish soon after the stimulation device is activated, and quickly return if the device is turned off. But surprisingly, there has been very little understanding of precisely why and how DBS works so well--a lack of knowledge that has held back efforts to further improve the therapy. Despite the great success of DBS, some significant problems remain. Customizing the stimulation delivered by DBS devices for each patient to maximally reduce symptoms is challenging and time-consuming. And a minority of patients never obtains the full benefit their physicians expect.
