Two additional important elements of the well-built clinical question to consider are the type of foreground question and the type of study (methodology). This information can be helpful in focusing the question and determining the most appropriate type of evidence. Foreground questions can be further divided into questions that relate to therapy, diagnosis, prognosis, etiology/harm Therapy: Questions of treatment in order to achieve some outcome. May include drugs, surgical intervention, change in diet, counseling, etc. Diagnosis: Questions of identification of a disorder in a patient presenting with specific symptoms. Prognosis: Questions of progression of a disease or likelihood of a disease occurring. Etiology/Harm: Questions of negative impact from an intervention or other exposure. Knowing the type of foreground question can help you select the best study design to answer your question. You always want to look for the study design that will yield the highest level of evidence. About sodium picosulfate Type of medicine Stimulant laxative Used for Constipation and before some surgery or medical examinations Also called For constipation: Dulcolax Pico For before medical procedures: CitraFleet; Picolax Available as Capsules, oral solution, sachets of powder Constipation can be caused by a poor diet, not drinking enough water and not going to the toilet as soon as you feel you need to. Pregnancy, a lack of exercise or movement (such as being ill in bed) and some medicines, including some painkillers, can also cause constipation. However, many people take laxatives when they do not need to because they believe that they are constipated unless they go to the toilet every day. This is not the case. Telehealth technology has been found to be an effective tool in reducing long standing queues at public hospitals. But the diffusion of telemedicine remains a dream in resource poor communities where power black outs prevail. The PICOMEDICINE Initiative is the answer. 2. Who gains the most? Mothers, young children and the elderly benefit the most. But the initiative will further try to revolutionize healthcare delivery in developing communities so that it benefit all. 3. Who pays? PICO is a method of putting together a search strategy that allows you to take a more evidence based approach to your literature searching when you are searching bibliographic databases like Medline (OVID), PubMed and Embase. PICO stands for: Patient/Population Who or What? Intervention How? Comparison What is the main alternative? (If appropriate) Outcome What are you trying to accomplish, measure, improve, effect? A preganant woman with type 2 diabetes is concerned about the effect her current treatment may have on her pregnancy and unborn child. The GP has heard that insulin pump therapy may be a more succesful treatment than conventional insulin therapy. However, the GP wants to get his facts right, so searches the literature. Bach promises big returns with ‘City for Champions’ plan

Mayor Steve Bach this week announced an ambitious plan totalling roughly $218 million to build four major attractions in Colorado Springs, without any new taxes needed. They include two downtown sites: a U.S. Olympic museum and a baseball stadium and event center (which sounds a bit like the convention center voters have refused to fund several times in the past). The other two are a University of Colorado at Colorado Springs sports medicine and performance center and an Air Force Academy visitor center outside the base's gates. Bach says the city will file its privately crafted application, dubbed "City for Champions," through a program under the Regional Tourism Act, which is designed to fund projects that will attract more out-of-state visitors. A state panel will assess the application and decide by December. Calling the projects a "catalytic spark" to ignite growth and create jobs, Bach says the money will come from state sales taxes rebated to the city ($82 million), private investment ($61.4 million), and from a variety of public sources ($74.6 million), including city borrowing.
