Heartbroken family’s tribute to ‘brilliant dad’ who died choking on food – Hull Daily Mail

A heartbroken family has paid tribute to a brilliant father and grandfather who sadly died after choking on his evening meal at a Hull nursing home .

David Wainberg, 75, went into cardiac arrest after he choked on a piece of food at Windermere House on June 20, 2018.

He sadly passed away at Hull Royal Infirmary three days later.

At an inquest into his death at Hull Coroners Court, his family said he was the life and soul of the party who will never be forgotten.

His eldest son, Mark, said: Although he suffered with mental health he was a brilliant father and grandfather.

He was a character. The life and soul of the party.

He will never be forgotten. He was much loved by his family, especially his grandchildren.

Mr Wainberg was a father to three sons, Mark, David and Jonathan, and had four grandchildren, Chloe, 9, Nathaniel, 6, Jacob, 3, and Felix, 1.

His family recalled his love for sport and said that if he had not been in a severe car accident at the age of four, where he almost lost a hand and foot, he would have gone on to become a professional sportsman.

His wife, Eve, said: Davids injuries at the time were considered to be life threatening and he went through two years of hospital treatment.

David still lived his life to the full playing football, cricket, rugby and various other sports.

He was a sportsman and a scholar and he enjoyed a good social life. This continued throughout Davids life.

He used to take his boys cycling, swimming, to the football and rugby and he even took them to Wembley Stadium. David did all he could with his sons and he was a really great dad.

He had a good circle of friends in Greatfield. His friends supported him right up until the day he died and they all visited him in hospital right until the end.

Mr Wainbergs mental health deteriorated in 2011 and due to very complex mental health issues he was admitted to Windermere House in east Hull three years later.

On June 20 last year, he had sat down to a burger as part of his evening meal before picking up a sweet bun from the table for dessert.

After finishing his meal and leaving the table, staff members noticed that he was still chewing his food and the alarm was raised after it appeared that he was choking.

Mr Wainberg lost consciousness and went into cardiac arrest. Staff members relentlessly tried to save his life until paramedics arrived.

He was rushed to Hull Royal Infirmary where a CT scan found he had suffered a hypoxic brain injury as a result of the choking incident.

Three days later, the decision was made to withdraw life support treatment as hospital staff found it was unlikely Mr Wainberg would recover. He sadly died on June 23.

Christopher Cooper, unit manager at Windermere House, told the court that there were four separate incidents since 2015 where Mr Wainberg had difficulty with his food.

However, he added that the nursing home carried out choking assessments on a monthly basis on all patients and that Mr Wainberg was consistently marked as a low risk.

Mr Cooper said: He spent a lot of time in his room and would come out for his meals and he would usually try and eat quickly to go back to his room.

We were aware that from the previous history there had been episodes where he appeared to struggle with his food and because of this we had a support plan in place where staff observed him at all times.

A root cause report conducted by hospital director Sally Mowforth found that staff members were relentless in trying to save Mr Wainbergs like and that a fantastic care plan was in place for him.

A spokesperson for Barchester Healthcare, who run Windermere House, said: "The team at Windermere House offerourdeepest condolences to the family of our patient.

"The safety and wellbeing of our patients is our utmost priority,and we worked with the relevant authorities to assist throughout their investigation.

"Our thoughts are with the family and friends at this very difficult time, and we will continue to do everything we can to support them.

Mr Wainbergs family repeatedly told the court that there was no doubt in the care at Windermere House and that staff members were like an extended member of the family.

Mark said: At Windermere we knew he was safe there. It was the right place for him.

They used to do quizzes, Sudoku and watch TV programmes with him. The staff were like an extended member of our family.

They came to weddings and other family events and they would even come over at Christmas.

We had a really good rapport with them. It is just unfortunate that this happened.

A jury at Hull Coroners Court concluded that Mr Wainbergs death was the result of an accident.
